Oozezi Creative Collective, Guardian Consent
1. Who this is forThis form is for the parent or legal guardian of anyone under 18 who wants to take part in something on Oozezi Creative Collective ("Oozezi", "we", "us"). Our
Terms & Conditions require a participant under 18 to have their guardian's express permission, this form is how that permission is given and kept on file, once per programme your ward wants to join.
2. You confirm you are the guardianBy signing, you confirm you are the parent or legal guardian of the minor named on this form, with the authority to consent on their behalf, and that the information you have given is accurate.
3. What your ward agrees toYour ward's participation is still governed by Oozezi's
Terms & Conditions and, where applicable, that programme's own rules, in full. Signing this form does not replace those Terms, it confirms you have read them and permit your ward to be bound by them.
4. Publishing your ward's workIf your ward's piece is accepted, Oozezi may publish it on the website and share it, in full or excerpt, on our social media channels, crediting your ward by the name or pen name given at submission, exactly as described in our
Terms & Conditions.
5. Video and photo consentSome programmes, such as The Last Word, require a video of the participant to be posted publicly (for example to Instagram) as part of taking part. Where that applies, signing this form means you specifically consent to your ward appearing in that video and to its public posting, in addition to the general publishing licence above.
6. Prizes and paymentWhere your ward's entry wins a prize, we will pay it to you, the guardian, on your ward's behalf, since a minor cannot usually hold the bank details a payment requires. You will be asked for your own payout details at that point, not your ward's.
7. How we will contact youWe will communicate about your ward's participation with you, the guardian, using the contact details on this form, not by arranging anything with your ward directly. Please keep an eye on that email and phone number for updates.
8. Your right to withdrawYou may withdraw your ward from a programme at any time by emailing us. There is no penalty for withdrawing, though anything already published under the licence above stays published unless we agree otherwise.
9. Your ward's informationThe details on this form are used only to consider and administer your ward's participation, and are handled as described in our Privacy Policy, which applies here too.
